Enrollment Information for John F Kennedy Day School
We are delighted you are considering enrolling your child at John F Kennedy Day School. Below is all the necessary information regarding our enrollment policies to ensure a smooth and successful enrollment process.
Admission Requirements
- Eligibility: Students who meet the Code of Federal Regulations requirements are eligible to attend John F Kennedy Day School.
- Tribal Membership: Students must be a member of a federally recognized tribe. This can be established through:
- Documented Indian eligibility, such as a Certificate of Indian Blood or Tribal membership card.
- Documentation verifying descent from an enrolled member, including familial tribal enrollment documents.
- Immunization: A current immunization record is mandatory for enrollment. All children must have state-required vaccinations.
- Enrollment Priority: Priority for enrollment is given to students residing in Cedar Creek and Carrizo.
Age Requirements
- Kindergarten: Students must be 5 years old by September 1 of the enrollment year, in accordance with Arizona Education Code 15-821.
- State Statute: A copy of the state statute regarding age limits is available upon request.
Attendance Area
- Service Area: John F Kennedy Day School serves elementary students on the White Mountain Apache reservation.
- Transportation: Students outside JFK bus routes may be accepted, but families are responsible for transportation to the end of the JFK bus routes.
Enrollment Forms
- Completed Forms Requirement: Students are considered enrolled when all required application forms are completed and received by the school. No student may attend classes until all forms are received.
- Required Forms: Necessary documents include:
- Student Enrollment Application
- Copy of Birth Certificate (Baptismal record or hospital certificate accepted)
- Copy of guardianship papers (for students not living with their parent)
- Copy of Social Security card
- Copy of current immunization record
- Primary Home Language Survey
- Consent Form for Field Trips and Competitive Sports
- Consent Form for Health Services
- Withdrawal form from previous school and Records Release Form (for transferring students)
- Internet and E-Mail Authorization Form
- Permission to publish student pictures
- Emergency Information and Check-out Form
- Parent/student handbook signature page
- Parent/Student/Teacher Partnership Compact Form
- Allergy verification signed by physicians as required by the ADE.
