Parent/Guardian Notification

Ensuring the well-being of our students is our top priority. If a student gets injured or becomes ill during school hours, we will promptly notify the parent(s) or guardian(s) listed on the emergency contact form. A parent or guardian needs to pick up the student as soon as possible. In severe cases, we will call an ambulance to transport the student to the Whiteriver Indian Health Services per the Medical Permission Card on file.

Parents must inform us of any serious or chronic medical conditions their children have. If we are unaware of these conditions, we cannot be held responsible for any injuries or illnesses resulting from these conditions.

Medication at School

When your child needs to take medication during school hours, please follow these procedures to ensure their safety and proper care:

  • Notification: We encourage parents/guardians to inform the front office in writing or by phone when their child brings any new medication or refills to school.
  • Administration: Upon arrival at school, your child should take all medications, whether prescribed or over the counter, to the front office. School personnel will administer the medication.
  • Storage: We will securely store medications in a locked office vault.
  • Labeling: Make sure the medication container is clearly labeled with the physician’s name, the student’s name, the drug name, and the correct dosage schedule.
  • Special Medications: Give EpiPens and inhalers directly to the teacher for immediate access.

Written Consent

For the safety and legal protection of all involved, we require the following consent procedures:

  • Parent Consent: Parents must provide written consent for any medication school personnel administer. This consent must include a telephone number to always reach the parent/guardian.
  • Liability: The consent form must release us from any responsibility if we do not administer the medication on time or if the child experiences an adverse reaction.
  • Emergency Contact: If the child experiences an adverse reaction to the medication, we will immediately call the phone number provided by the parent(s)/guardian(s).
